Insurance Benefits
It is important to contact your health insurance carrier, as well as our office, to confirm participation with your insurance plan. It is our policy to verify your insurance coverage before your appointment. At that time it will be confirmed if we participate with your insurance carrier. At the time of service, you will be asked to pay your co-pay, deductible, or co-insurance portion. You will need to bring your photo ID and insurance card to your appointment. Please review the information below to learn more about in-network and out-of-network insurance plans.

In-Network

Out-of-Network
Often there is a network symbol on the back of your insurance card that enables you to receive health care services out-of-network. We will work with you and your out-of-network benefits or provide a fee schedule for you to see our physicians on an out-of-network basis. If we are out-of-network with your insurance plan, payment in full is expected at the time of service. We will be happy to provide you with the information necessary for submitting your claim to your insurance carrier. If you would like to discuss billing options, please call our Billing Department at 561-694-7776, option 4. The billing department is available to take your call Monday – Friday between 8:30am and 5:00pm. We participate with the following networks:

Self-Pay
We also provide medical care to self-paying patients. We will be happy to provide a fee schedule for you to see our physicians on a self-pay basis.
Referrals/Authorizations
If a referral or authorization is required by your plan, you are responsible for contacting your Primary Care Physician (PCP) to start the process.
