Listed below are insurance plans accepted at PBOI. Our insurance carriers change frequently. Please contact us if your insurance is not listed. If you have any questions regarding your insurance, please contact us at (561) 694-7776. We also provide medical care to self-paying patients.

While most of our physicians participate with all these plans, it may vary. It is important to contact your health insurance carrier, as well as our office, to confirm participation.

It is our policy to verify your insurance coverage, whenever possible, before your appointment. At that time it will be determined if we participate with your insurance carrier. If so, we will be happy to bill your carrier for our services. You will be asked to pay your co-pay, deductible, or co-insurance portion at the time of service.

If a referral or authorization is required by your plan, you are responsible for contacting your Primary Care Physician (PCP) to start the process. If we do not participate with your insurance plan, payment in full is expected at the time of service.

We will be happy to provide you with the information necessary for submitting your claim to your insurance carrier. Our Accounts Receivable Department will be happy to work with you on any billing or payment problems.
