Patients - Rehab Department Forms

To ensure that our patients reach the highest possible level of function after an injury or surgery, PBOI offers rehabilitation services provided by a professional and compassionate staff. Our modern facilities combined with our dedicated staff, provide the patient with the optimal opportunity to have a successful rehab experience.

Palm Beach Orthopaedic Institute's Rehabilitation Department has two offices to serve your injury or surgery rehabilitation needs. Our primary center is at the Palm Beach Gardens Medical Pavilion, 3401 PGA Blvd. We also have clinic space in our office at the Jupiter Medical Center's Out-Patient Surgery Center, Medical Park Building at 2055 North Military Trail, Jupiter.

Our Rehabilitation Department staff is composed of experienced, licensed health care providers including physical therapists, athletic trainers and physical therapy assistants.

We specialize in helping you recover from orthopedic surgery of all varieties. Total knee replacement, anterior cruciate ligament reconstruction, knee and shoulder arthroscopy, and lumbar microdiscectomy and laminectomy are just a few of the conditions that we see on a routine basis.

We can also train you in the proper exercises to help you recover from ankle sprains, plantar fasciitis, shoulder dislocation, tennis elbow, and anterior cruciate ligament injury.

We treat all types of musculoskeletal problems, whether caused by sports participation or activities of daily living, ranging from acute neck and low back sprains to problems related to osteoarthritis.
