Medical Records
To speak with someone in the Medical Records Department, please call the office at (561) 694-7776, option 5. They are available to take your calls Monday – Friday between 8:30am and 5:00pm.
Request Medical Records
If you would like to request medical records, you must print, complete and fax the Medical Records Release Form to (561) 727-1401. PBOI considers all patient records to be confidential and will release copies of the patient’s medical records only with the patient’s written approval.

There is a small fee of $1.00 per page for the first 25 pgs. and $.25 for each page thereafter. Please allow 3 to 5 days for records requests. All patients are required to complete a Medical Records Release Form (HIPAA compliant) which authorizes PBOI to release protected health information.
A patient’s original X-Rays remain part of the permanent medical record. Copies of X-Rays may be produced for a nominal fee to cover the cost of new X-Ray film.
